Tibial avulsion fracture8/24/2023 Even if a fracture does extend to the physis, it does not make it a Salter-Harris fracture. 1 Typically, tibial eminence fractures are associated with falls from bicycles, where an excessive force is exerted on the ACL causing a bony avulsion instead of an acute ACL tear. Pediatric patients are most commonly affected due to the weakness of the subchondral bone, which causes the bone to fail before the ACL. Although tibial eminence fractures can occur at any age, most occur between the ages of 8 and 14 years. Most commonly, these fractures are found in the pediatric population and rarely occur in adults. Not all metaphyseal fractures intersect the physis. Tibial eminence fractures are not common injuries. They may occur in pediatric or adult patients. It is named after Paul Jules Tillaux, French surgeon and anatomist (1834-1904) 2. Metaphyseal fractures are fractures that involve the metaphysis of tubular bones. ComplicationsĪs with any intra-articular fracture, a persistent articular surface step predisposed the joint to premature secondary osteoarthritis. Operative reduction and internal fixation (ORIF) is required when the displacement is marked or unable to be corrected with closed reduction. The degree of displacement will dictate management. The lack of a metaphyseal fracture component in the coronal plane (evaluated with lateral x-ray or CT) distinguishes a Tillaux fracture from a triplanar fracture. Vertical fracture through the distal tibial epiphysis (Salter-Harris III) with a horizontal extension through the lateral aspect of the physis. Tibial tuberosity avulsion fracture usually occurs in younger dogs due to the area of the tibia not being fully fused to the rest of the bone. The fracture requires an open physis (the lateral aspect of the distal tibial physis usually closes between 12 to 15 years of age while the medial aspect closes earlier). The lateral epiphyseal involvement is due to growth plate fusion commencing from medial to lateral aspect. This type of fracture usually requires surgery for a good outcome, but in some cases can be managed conservatively. This region of the bone is susceptible to developing a fracture in puppies and kittens. With this mechanism, the inferior anteroinferior tibiofibular ligament avulses the anterolateral corner of the distal tibial epiphysis 3. Bone growth comes from a soft layer of tissue at the ends of the bones called growth plates. The fracture commonly results from an abduction-external rotation mechanism.
